Organizational
Health Inc. recognizes the importance of privacy and the sensitivity
of personal information. We are committed to protecting all personal
information we hold. For these reasons we have created the following
Privacy Policy. This Privacy Policy outlines how we manage your
personal information and safeguard your privacy. We take great care
to keep your personal information confidential and secure.
Effective
January 1, 2004, all businesses engaged in commercial activities were
required to comply with the Personal Information Protection and
Electronic Documents Act (PIPEDA). The Act gives you rights
concerning the privacy of your personal information. Our policy is
designed to comply with both provincial and national privacy
legislation.

What is Personal Information?
Personal
Information is defined as any information about an identifiable
individual, but does not include the name, title or business address
or telephone number of an employee of an organization. In addition,
some information that is otherwise publicly available is not
protected by the Act.
For example,
your personal information includes any information that relates to
your physical or mental health, race, origin, colour, religion, age,
marital status, education, medical, criminal or employment history,
information relating to financial transactions, identifying number,
an unlisted address or unlisted telephone number, fingerprints, and blood

How we collect, use and disclose personal
information.
1. We will only
collect information from you for the purposes identified to you.
We collect this
information so that we can provide you with our best possible
healthcare advice and assistance.
2. Why do we
collect this information?
Your personal
information is important so that we can understand your needs,
determine your eligibility for our services, and provide you with the
assistance to which you’re entitled. Our service includes but
is not limited to employee and family assistance counselling, claims
adjudication, disability management, return to work planning and
implementation, and medical service facilitation.
This helps us
meet contractual requirements relating to the services we provide.
However, we will make sure that the purpose for collecting this
information is clearly outlined to you.
3. We will
always ask for your permission
When you seek
eligibility for our services, we will ask for your consent to obtain
personal information. When you have a disability claim, your consent
will be requested when you complete the claim application. We will
then rely on this consent to access information needed for your
claim. You may withdraw your permission to use, collect or disclose
this information at any time, understanding that this may affect the
services you receive from OHI.
When we obtain
medical information about you, we will not disclose that medical
information to your employer without your consent.
4. We will keep
your information only for as long as required
We have legal
requirements to keep personal information, including health
information for a specific period of time. Once that timeframe has
ceased, your information will be safely destroyed.

Health Inc. takes all reasonable precautions to ensure that your
personal information is kept safe from loss, unauthorized access, modification
or disclosure.
Steps
taken to protect your information include:

with supervised alarms, and locked cabinet storage;
- restricted file
access to personal information;
- deploying
technological safeguards like passwords, security software and
firewalls to prevent hacking or unauthorized computer access;
- electronic
information is transmitted either through a direct line or is
made anonymous or encrypted;
- internal password
and security policies.

Your rights associated with your personal information.
1. You have the
right to ask what information we hold about you, and to request to
view that information, as provided by law. OHI may choose not to
release medical or psychological information if we believe the
release may cause the individual to harm themselves or another
person. In these situations, OHI may manage the disclosure through
another person (for example, a physician or psychologist) or refer
the person to the source (for example, the doctor, psychologist, or
other physician) who provided the information.
You have the
right to ask how we are using this information, and to whom it may
have been disclosed.
Information
discussed with your counselor or care manager will be treated as
confidential unless any of the following apply:
a) if you have
signed a release of information for a specific person
b ) if, in the
professional opinion of your counselor or care manager, there is a
potential risk to you or others
c) if there is a
legal obligation to report; for example, cases of child abuse .
d) if the
counselor or care manager is legally required by a court of law to
testify or submit a report or release records.
2. If you
believe that the information we hold about you is incorrect or
incomplete, you have the right to ask us to change it. We will make
the necessary changes.
